2. Name and occupation of related parties, whether member(s) accused of misconduct, victim(s), and witness(ies);
3. Description of the fact, including possible details such as repetitive actions, consequences, and values, when applicable;
4. Appended Documents, when applicable, such as e-mails and photos;
5. Name and e-mail of the whistleblower, in case he / she chooses not to send an anonymous report.
